CDC serves as the national focus for developing and applying disease prevention and control, environmental health, and health promotion and education activities designed to improve the health of the people of the United States.

The Centers for Medicare and Medicaid is the federal agency that administers Medicare, Medicaid, and the State Children's Health Insurance Program (SCHIP). Community Solutions Social Indicators Reports: Community Health
The Social Indicators Reports provide descriptive information about people, their characteristics and circumstances. Researchers explain the study is relevant to an issue which can be affected by social policy, supplying information that is measurable, has the same meaning over time, utilizes consistent methods of data collection and has readily agreeable meaning. The Social Indicators Reports provide information on the geographic level of social planning areas (neighborhoods) in the city of Cleveland and all the suburban communities in Cuyahoga County.

Community Voices: HealthCare for the Underserved, a multi-year initiative funded by the W. K. Kellogg Foundation, is designed to improve access to quality health services for the underserved--the working poor, individuals or families who receive public assistance and those who lack any or adequate health insurance. Federal Department Portal for Health Issues
The federal government has created a portal to the web sites of a number of multi-agency health initiatives and activities of the U.S. Department of Health and Human Services and other Federal Departments. Healthy People 2010
Healthy People 2010 is the prevention agenda for the Nation. It is a statement of national health objectives designed to identify the most significant preventable threats to health and to establish national goals to reduce these threats. The Healthy People 2010 site explains health choices, describes leading health indicators, and provides publications and data. The site also includes healthfinder®-- the U.S. government gateway to reliable health information resources that have been carefully selected by the U.S. Department of Health and Human Services from over 1,700 government agencies and nonprofit organizations.

Kaiser Family Foundation
The Henry J. Kaiser Family Foundation is an independent philanthropy focusing on the major health care issues facing the nation. The Foundation is an independent voice and source of facts and analysis for policymakers, the media, the health care community, and the general public. National Center for Health Statistics (NCHS)
The NCHS is part of the Centers for Disease Control and Prevention. NCHS data systems include data on vital events as well as information on health status, lifestyle and exposure to unhealthy influences, the onset and diagnosis of illness and disability, and the use of health care. These data are used by policymakers in Congress and the Administration, by medical researchers, and by others in the health community.

The Robert Wood Johnson Foundation is the largest US foundation devoted to improving the health and health care of all Americans. The site contains a list of RWJF publications, an extensive resource center in three areas: end-of-life care, chronic conditions, and substance abuse and comprehensive index of web sites organized by areas of interest.

U.S. Department of Health and Human Services Health Resources and Services Administration (HRSA)
The mission of the HRSA is to improve the nation's health by assuring equal access to comprehensive, culturally competent, quality health care for all. The site provides explanations of HRSA programs, data and statistics on major health issues in the U.S., HRSA publications, also contains extensive information on the Community Health Indicators Project which provides profiles of each county's overall health status using a broad spectrum of health indicators.

Vision USA
VISION USA, now open year-round, is asking your assistance in locating low-income, uninsured, working families who may qualify for FREE eye care through VISION USA.
Currently, over 7,000 optometrists volunteer their professional services through VISION USA. Since 1991, this program has served over 300,000 low-income working Americans and hopes to serve many more by accepting applications 12 months a year.
